UMFS is seeking a full-time employee in our Alexandria, VA office to provide Intensive Care Coordination services to youth and families in the community.
Requires day and evening availability.
BSW/related degree required.

Intensive Care Coordinator provides numerous essential support services to clients and their
caregivers/families. Working with youth and families to create teams, develop goals,
plan/coordinate transitions, and monitor progress to ensure that youth can maintain in the
community with their family.

Implement the 10 principles of High Fidelity Wraparound: family voice and choice,
outcomes-based, strengths-based, culturally competent, natural supports, individualized,
community-based, persistence, team-based, and collaboration.

Implement the 4 phases of High Fidelity Wraparound: engagement, planning,
implementation, transition. Work with families to develop natural supports in
their communities.

Identify and link youth and families to natural and community resources.

Conduct initial and discharge assessments to include strengths and needs assessments
of the youth, family, extended family, and the community.

Provide written and verbal reports (as required) to the FAPT for ongoing monitoring.

Identify immediate and/or potential crises; develop and ensure implementation of a
safety plan as needed.

Engage, support, and facilitate conversations with family, youth and team members
regarding service planning and implementation.

Review, discuss and update the family meeting action plan on an as needed basis in
collaboration with team members.

Other duties as assigned.

Bachelor’s degree (BSW) in social work or related degree with two years of experience
providing mental health services to children with a mental health diagnosis. MSW or
related preferred.

Demonstrated ability to apply sound clinical casework and counseling skills to children
and families, understand treatment modalities and their implications for services to families
and youth. Completion of “High Fidelity Wraparound” training preferred.

Availability to work days and evenings when required.
